Dental Insurance
A beautiful smile is almost always the result of both a commitment to dental health and a financial investment. To help our clients get the beautiful smile they deserve, we offer an in-house affordable dental insurance plan. This private plan is tailored around the services our customers need most - such as cleanings, fluoride treatments, and dental imaging - and offers options for both individuals and families. It does not disqualify pre-existing conditions, and there is no waiting period for acceptance. In addition, subscribers have access to our Preferred In-house Discounted Fee Plan. Our in-house plan is simply one of the best dental plans in the Parma Heights area.
Individual Dental Plan – Adult: $175 annual premium
- *Regular Exams (2 per year)
- Bitewing X-ray (1 per year)
- Panoramic X-ray (1 per year)
- Adult Cleanings (3 per year)

Individual Dental Plan – Child: $100 annual premium

2016 Extended Enrollment period ends March 31. Standard enrollment periods begin December 1 and end January 1 of the following year. Premiums are due in-full by January 5. All fees for additional services are due in full at the time of service.
*A regular exam is a routine check-up performed in conjunction with a cleaning.
**An emergency exam is any exam done to address an acute problem such as pain, swelling, infection, or broken tooth. Emergency exams DO NOT INCLUDE emergency treatment. Emergency treatment to relieve pain or repair broken teeth will be charged accordingly.
Fees for full-mouth reconstruction cases will be determined based on need and case specifics.
Patients covered under other dental insurance plans are not eligible.

A few notes about dental insurance:
Please note that dental insurance rarely covers the entire cost of your treatment, and you are responsible for any and all fees associated with your treatment. If you’d like us to bill your insurance company for their portion, please alert the staff prior to treatment. The remaining fees are due in full at the time of treatment. Any portion left unpaid by your insurance company after 60 days becomes your responsibility.
Having insurance does not constitute a payment plan. If you need to make payments, please alert our staff prior to treatment. Payment plans are available through CareCredit. All payment plans must be agreed to in writing prior to the beginning of treatment.
